Removing an Alternate Address

This form is to be completed by parents/guardians requesting that an alternate address be removed from their student’s transportation file. Once the alternate address has been removed, transportation arrangements will be based on the student’s home address, subject to transportation eligibility and routing requirements. Please note that changes to transportation information may require processing time before they are reflected in the BusPlanner Parent Portal.
